This form is for agencies who have made a formal notification to the Privacy Commissioner to provide updated information. This form is not to be used for an initial notification.

Use this form when making a formal application to access government information held by a NSW public sector agency, other than the IPC.

You can use this form and send to a NSW public sector agency to make a privacy complaint and request an internal review.

Use this form if your complaint concerns the conduct (action or inaction) of an agency in exercising its function under the GIPA Act.

Use this form if you are applying for an external review by the Information Commissioner of a reviewable decision under the GIPA Act.
